Bridge to Home Rehabilitation Program

Sub-acute, short-term rehabilitation services are available at our Grover M. Hermann Hospital Division in Callicoon, New York through our Bridge to Home Rehabilitation Program. We provide patients with the quiet, soothing environment needed for healing. Our expert staff of registered nurses can provide the clinical expertise to get you back on your feet and ready to go home.

Who needs the Bridge to Home Rehabilitation Program?
The person who:

  • Is recovering from major surgery, including joint replacement
  • Has experienced repeated hospitalizations
  • Is recovering from a stroke
  • Is recuperating from a major accident
  • Needs help managing pain
  • Requires wound care or amputation care
  • Is coping with congestive heart failure
  • Is gradually getting better from a serious condition
  • Needs enteral nutrition

To qualify for the Bridge to Home Rehabilitation Program, patient must:

  • Be under the supervision of professional personnel
  • Require skilled services on a daily basis
  • Have a 3-day acute admission prior to entering the Bridge to Home Rehabilitation Program
