What Is A Geriatric Assessment?

A Geriatric Assessment is a comprehensive medical examination that is completed by a physician who is board certified in geriatrics, as well as a registered nurse, dietitian, and social worker. The Geriatric Assessment evaluates the whole person, not just the disease process.

The Dementia Evaluation

Assess a person’s symptoms:

• Memory Loss
• Disorientation
• Confusion
• Poor Concentration

Establish a diagnosis

Make recommendations that assist the person with Alzheimer’s or other dementia, and their family and caregivers to plan for:

• Present Needs
• Future Needs

How Is My Family Doctor Involved In The Geriatric Assessment Process?

Your medical history will be obtained from your doctor(s). This information will be used by our staff to assure that we have all the necessary information to provide you and your doctor with suggestions on how you can remain healthy and active.

If I Am Caring For An Elderly Parent Or Spouse, How Can The Geriatric Assessment help?

The Geriatric Assessment social worker will assess the patient’s eligibility for needed services, such as:

• Personal Care Services
• Medicaid
• Durable Medical Equipment
• Adult Day Care
• Personal Emergency Alert System

The Geriatric Assessment Consists of Two Evaluation Days

Day One - Four Hours

• Medical history
• Falls Assessment
• Dementia and Depression Evaluation
• Diet and Medication Assessment


Day Two - One Hour

• Comprehensive physical examination by geriatrician

The patient, caregiver and doctor will receive a comprehensive geriatric assessment report with findings and suggestions on treatment and needed services.
