Medical records are confidential, but you may arrange access to your own records by contacting the Medical Records Department.

You may also obtain copies of your medical records by submitting a completed and signed Patient Request for Release of Medical Records to the Medical Records Department.

The Medical Records Department is located on the first floor of the main building in Harris. The department is open from 8:30 a.m. to 6:00 p.m., Monday through Friday, and from 8:30 a.m. to 4:00 p.m. on Saturday and Sunday.
